Sleep disturbance and mild cognitive impairment (MCI) frequently occur together in older adults, and both are recognized early markers for dementia risk. A new study from Emory University's Nell Hodgson Woodruff School of Nursing, published in Sleep Medicine, suggests a widely available supplement may be worth investigating as a modifiable factor in that overlap: high-dose vitamin D.
A Notable Gap in Test Scores
The study examined 54 older adults who had both diagnosed sleep disturbance and mild cognitive impairment. Researchers compared cognitive performance, measured with the Montreal Cognitive Assessment (MoCA), between participants who reported taking 5,000 IU or more of vitamin D daily and those who reported taking none.
Participants in the high-dose vitamin D group scored more than 13% higher on the MoCA than those who weren't supplementing — a difference researchers describe as clinically notable given how narrow the margin between normal cognition and impairment can be on this test. The work was funded by the National Institute on Aging.
Why Sleep and Cognition Are Studied Together
Sleep disturbance and MCI are both established early-window risk factors for progression to dementia, and there's growing evidence that poor sleep and cognitive decline can accelerate one another — disrupted sleep impairs the brain's overnight clearance of metabolic waste, including proteins implicated in Alzheimer's disease, while cognitive decline itself can further destabilize sleep architecture. That reciprocal relationship has made researchers increasingly interested in interventions that might act on both sides of the cycle at once.
Vitamin D has known roles in neuronal health and has previously been associated with sleep regulation in observational research, making it a reasonable candidate to study in this specific population — though this analysis focused on cognitive outcomes rather than measuring change in sleep quality directly.
A Preliminary, Not Definitive, Finding
The study's authors were explicit that this is a preliminary, cross-sectional analysis rather than a randomized controlled trial. With only 54 participants and reliance on self-reported supplement use, the findings can't establish that vitamin D caused the better cognitive scores — people who supplement consistently may differ from those who don't in other health-relevant ways the study didn't measure. The researchers frame the results as justification for a properly randomized, controlled trial to test whether vitamin D supplementation actually improves cognition in this population, not as evidence to act on yet.
What This Means for Patients
For older adults already dealing with sleep problems and early memory concerns, this study is a signal worth discussing with a physician rather than a reason to start high-dose supplementation on their own. Vitamin D deficiency is common and easily checked with a blood test, and correcting a genuine deficiency carries well-established benefits for bone and general health regardless of any cognitive effect. But 5,000 IU daily exceeds standard recommended intake for most adults, and sustained high doses can cause toxicity, so any decision to supplement at that level should happen under medical supervision — particularly while researchers pursue the randomized trial needed to know whether this association reflects a real, causal benefit.
