Restless Legs Syndrome May Be a Sensory Processing Disorder, Not Just a Motor One, New Study Finds

Restless legs syndrome is defined, clinically, by an urge to move the legs — a motor symptom. A new study published in Brain and Behavior argues that framing may be incomplete, and that RLS is better understood as a broader disorder of how the nervous system processes sensory information generally.

Researchers led by Özgül Ocak at Çanakkale Onsekiz Mart University in Türkiye assessed 81 patients diagnosed with RLS using Dunn's Sensory Processing Model, a validated framework originally developed to characterize how individuals register, respond to, and regulate everyday sensory input — everything from background noise to clothing textures to crowded rooms.

A Distinct Sensory Signature

Compared with normative population data, RLS patients scored significantly higher on sensory sensitivity, sensory avoidance, and low registration — a pattern indicating both heightened awareness of sensory input and a tendency to notice less of it before reacting strongly once a threshold is crossed. They scored lower on sensory seeking, the tendency to actively pursue additional sensory stimulation.

In other words, people with RLS weren't just experiencing an isolated crawling or tingling sensation localized to their legs at night. Their sensory processing profile, measured across everyday contexts unrelated to sleep or their legs, looked different from the general population's — and consistently so, across multiple dimensions of the Dunn model.

The Pattern Tracks With How Bad the Disease Gets

The strongest signal in the data was sensory avoidance: it was significantly associated with both disease severity and how long a patient had lived with RLS. Patients with more severe symptoms and longer disease duration showed more pronounced avoidance behavior — actively steering away from sensory situations that felt overwhelming.

That correlation doesn't prove which way causation runs. It's possible that years of RLS symptoms reshape how the nervous system handles sensory input generally, a kind of sensitization that spreads beyond the legs over time. It's also possible that people with an underlying tendency toward heightened sensory processing are more prone to developing severe, persistent RLS in the first place. The cross-sectional design of this study can't distinguish between the two, but either interpretation reframes RLS as something more systemic than a localized nighttime leg symptom.

Why This Matters for a Condition Still Being Defined

RLS has historically been described through a motor and dopaminergic lens, and current first-line treatments — iron repletion, gabapentinoids, and specific device therapies — largely target that framework. A sensory-processing account doesn't contradict that treatment approach, but it does suggest sensory dimensions of the disorder may currently go unmeasured and unaddressed in standard clinical evaluation, which typically asks about leg symptoms rather than broader sensory sensitivity or avoidance.

If sensory processing patterns turn out to meaningfully predict who develops more severe or treatment-resistant RLS, they could eventually serve as an additional clinical marker — one measured with existing, validated questionnaires rather than new imaging or lab tests.

What This Means for Patients

This study doesn't change how RLS should be treated today. Iron evaluation and the other guideline-recommended therapies remain the appropriate first steps for anyone diagnosed with the condition.

But patients who feel that their RLS involves more than just leg discomfort — a broader sensitivity to noise, touch, or crowded environments, or a tendency to avoid situations that feel sensorially overwhelming — may be noticing something real and worth mentioning to their clinician, rather than an unrelated quirk. As researchers continue to map the sensory dimensions of RLS, that kind of detail could eventually inform how the condition is assessed and managed, particularly for patients whose symptoms have proven severe or difficult to control with standard approaches.
