For children with obstructive sleep apnea, a breathing machine can only help if they will actually wear it. That turns out to be one of the hardest problems in pediatric sleep medicine — and a team in São Paulo is building a structured program to solve it inside a public hospital serving families with few resources.
The study, published in Frontiers in Sleep by Ila Linares and colleagues at the Instituto da Criança of the University of São Paulo medical school (ICr-FMUSP), describes an interdisciplinary program to improve children's adherence to positive airway pressure (PAP) therapy. It was developed with collaborators at the Universidade Federal do Rio Grande do Sul and the Children's Hospital of Philadelphia, whose long-running pediatric CPAP program served as the model.
It is worth being precise about what this paper is: a study protocol. It lays out the design, rationale, and measurement plan for the program rather than reporting final outcomes. That makes it a blueprint — a detailed account of how a well-resourced approach can be adapted to a very different setting — with results to follow.
Why Kids Struggle With CPAP
PAP therapy delivers a steady stream of air through a mask to keep the airway open during sleep. For adults, the challenge is largely about comfort and habit. For children — especially young children, or those with developmental differences — the mask itself can be frightening or intolerable. A device strapped to the face at bedtime runs against every instinct, and a single bad experience can set back weeks of progress.
The result is that pediatric PAP adherence is notoriously low, and there is, as the authors note, "no consensus regarding the correct period of adherence to PAP treatment in the pediatric population." Untreated childhood sleep apnea, meanwhile, is linked to problems with growth, behavior, learning, and cardiovascular health.
A Step-by-Step Approach
The program, aimed at up to 20 children aged 1 to 18 with polysomnography-confirmed OSA, is built around a team of a sleep physician, a respiratory therapist, and a psychologist. Its core is a gradual desensitization process that treats mask acceptance as a skill to be learned, not assumed:
- Psychoeducation first: Families receive booklets and videos, and the team gathers each child's preferences to personalize motivation strategies.
- A five-step mask ladder: Children progress from holding the mask without headgear, to wearing it with headgear, to turning the device on, and finally to nighttime use — with structured daytime sessions of at least 15 to 20 minutes.
- Behavioral techniques: Praise, stickers and rewards, pairing the device with favorite activities, and gently extinguishing avoidance behaviors.
- Telehealth check-ins: Weekly video calls address barriers and celebrate progress between visits.
- Monthly in-person visits: The team downloads usage data from the device, checks the equipment, reviews side effects, and adjusts recommendations.
Success is measured objectively — the target is using the device at least four hours a night on 70% or more of nights — alongside sleep symptom scores, breathing measures, and changes in parents' confidence and motivation.
Built for the Real World
What sets the program apart is its honesty about context. Adapting a Children's Hospital of Philadelphia model to a Brazilian public clinic means confronting constraints that a well-funded center never has to: "low income profile, limited access to technology, cultural barriers affecting interaction with medical devices," and shortages of the interdisciplinary staff the program depends on.
The design tackles these head-on. Telehealth follow-ups cut down on costly, time-consuming travel. Parent-led exposure exercises at home reduce how often families need to come in person. And by running inside the SUS — Brazil's universal health system, "marked by resource limitations, heterogeneous patient populations, and high service demand" — the study earns what the authors call ecological validity: if it works here, it can plausibly work in many places.
Its goal is to provide an integrated clinical service, optimizing the time required for PAP adaptation, increasing adherence rates, and reducing costs associated with medical problems caused by untreated OSA.
What Comes Next
Because this is a protocol, the crucial question — does the program actually raise adherence? — is still open. The authors are candid that "further studies involving representative samples are essential to evaluate the effectiveness of PAP adherence programs and strengthen the generalizability of findings."
Still, the effort matters. Much of the world's research on pediatric sleep apnea comes from wealthy health systems, and the tools it produces do not always survive the trip to public clinics serving lower-income families. By starting from the constraints rather than ignoring them, the São Paulo team is testing something more useful than a program that works only in ideal conditions: one designed to work where most children actually receive care.
