Shift work has long been linked to higher rates of obesity, type 2 diabetes, cardiovascular disease, and metabolic syndrome, and researchers have generally pointed to circadian misalignment — the body's internal clock being out of step with a person's actual sleep and activity schedule — as the likely driver. A new analysis of Japan's 2018 National Health and Nutrition Survey, published this year in the journal Industrial Health, adds a specific and previously underappreciated piece to that picture: shift workers' diets look measurably different from those of people working standard daytime hours, in ways that track with their shorter, more disrupted sleep.
What the Data Show
Researchers Toshiko Yamazaki, Kazuko Ishikawa-Takata, and Hidemi Takimoto analyzed survey data from 1,814 working adults, comparing shift workers against daytime workers on measures of sleep duration and dietary intake across major food groups. Shift workers of both sexes reported shorter sleep than their daytime-working counterparts, consistent with decades of prior research on how rotating and night shifts fragment and shorten sleep.
The dietary findings were where the study broke new ground. Shift workers consumed significantly less raw seafood than daytime workers, a notable finding given fish's prominent place in the Japanese diet as a source of omega-3 fatty acids, protein, and other nutrients tied to cardiovascular and metabolic health. Among women specifically, shift workers also reported lower intake of milk and dairy products than female daytime workers — a gap not mirrored in the male comparison. Female shift workers additionally reported working more days per week on average than female daytime workers, a scheduling difference that did not appear between male shift and daytime workers, suggesting the burden of shift work's competing demands on time and routine may fall unevenly by sex.
Why Diet May Matter as Much as the Clock
The researchers frame these findings within the broader circadian-disruption model of shift-work-related disease: when work and sleep schedules pull away from a person's natural light-dark cycle, eating patterns tend to shift too, often toward foods that are quicker to prepare or obtain at odd hours rather than foods that require planning or fresh preparation, like raw fish or dairy staples. Because nutrient-dense foods such as seafood and dairy are harder to fit into compressed, irregular schedules, shift workers may be accumulating dietary gaps on top of the sleep debt already well documented in this population.
The study is cross-sectional, meaning it captures a single snapshot of workers' habits rather than tracking anyone over time, so it cannot establish that shift work directly causes these dietary shifts or that the dietary shifts themselves worsen shift workers' health outcomes. The authors note the differences in individual nutrients were often statistically significant but modest in size, and future work will need to test whether targeted nutritional support changes health trajectories for people on rotating or night schedules.
What This Means for Patients
For the roughly one in five workers worldwide who work some form of non-standard shift, this study is a reminder that managing the health effects of shift work may require more than trying to protect sleep hours alone. Workers on rotating or night shifts, and especially women balancing shift work with more workdays per week, may benefit from deliberately planning for seafood- and dairy-inclusive meals rather than defaulting to whatever is fastest or most available during odd-hour breaks — batch-prepared fish dishes or shelf-stable dairy alternatives, for instance, can help close gaps that are easy to overlook when the primary focus is simply getting enough sleep. Clinicians working with shift-employed patients may also want to ask about dietary patterns, not just sleep duration, when assessing cardiometabolic risk.
